First step make selection to your subject using the quick selection tool. Drag the selection to the skin and remove the areas. Go to select and refine edge. Check the radius and increase little bit the radius. Just little bit. Make a new layer with layer mask go to filter > blur & surface blend.
Radius: 16 pixels
Threshold: 12 levels
Choose your best combination of smoothness just to out the blemishes and imperfections. Continue until you're satisfied enough of your work.
